Patient Rights
In Our Practice, You have the Right to:
- Be treated in a fair, polite and caring manner.
- Know all the facts regarding the condition of your
oral health, including possible treatments and expected outcomes.
- Receive an estimate of fees prior to accepting treatment.
- Have access to documentation identifying how
we will use your medical information (HIPAA).
- See your own dental records and request a copy
for a small fee.
- Refuse treatment with an explanation of possible
health consequences in doing so.

Patient Responsibilities
In Our Practice, You have a Responsibility to:
- To obtain and carefully consider the details of your oral health and to weigh treatment decisions against the potential consequences of declining treatment.
- Notify us with any medical condition or medication that you are taking, regardless of how minor it may seem.
- Ask questions, express opinions or concerns regarding your care.
- Schedule appointments and arrive 15 minutes prior to your appointment time at each visit, or provide 48 hours' notice in the event of a need to cancel.
- Maintain the recommended schedule of preventative care so that our partnership with you in managing your oral condition remains as strong as possible.
